Application Form

Before filling out this application, please know that there is a legal form required to be signed by both parents, unless there is only one parent on the death record or if one of the biological parents is deceased; this form will be signed at the cemetery unless a funeral home is involved; in which case the legal documents will likely be signed at the funeral home.

Also know, if you choose cremation, we can still provide a place for your child using a vaulted urn.

Please Read: If your child lived outside of the womb for any period of time, there is another application that you will need to fill out. Please indicate “Live Birth” in the dropdown menu, and we will reach out to you for the next steps.
