Application Form

Before filling out this application, please know that there is a legal form required to be signed by both parents, unless there is only one parent on the death record or if one of the biological parents is deceased; this form will be signed at the cemetery unless a funeral home is involved; in which case the legal documents will likely be signed at the funeral home.

Also know, if you choose cremation, we can still provide a place for your child using a vaulted urn.
